Book online using the appointment form on our website.
Walk-ins: limited and based on availability—call first to check before coming in.
What to bring/prepare
Full name, date of birth, phone, and email.
Fill out completely out web registration that we send to you after you schedule.
Insurance cards (vision and medical) and any referrals.
Current glasses or contacts and any prescriptions.
Confirm and arrival
You’ll get confirmation and reminders (text, or email). Verify date, and time.
Arrive 10–15 minutes early. Bring ID, insurance cards, glasses/contacts, and a list of medications.
Expect 45–60 minutes for a routine exam; longer for fittings or special tests.
Need to reschedule or cancel? Call as soon as possible—fees may apply per our cancellation policy.
